I hereby acknowledge that it is my desire to participate in ministries and activities sponsored by Churches United including activities on and/or away from the church premises.

I am voluntarily participating in these ministries and activities, including transportation to and from such activities, if applicable, and accept any assigned duties with knowledge of the dangers involved and hereby agree to accept all risks of injury as a result of such participation.

I understand that I may appear in printed and digital media footage used by Churches United.

As lawful consideration for participating in such ministries and activities, I do hereby agree to indemnify and hold harmless Churches United, its officers, employees, volunteers and members of the Board of Directors from any and all claims, lawsuits, demands, causes of action, liability, loss, damage and/or injury, of any kind whatsoever (including without limitation all claims for monetary loss, property damage, equitable relief, personal injury and/or wrongful death) that I and my heirs, distributes, guardians, legal representatives or assigns, now have or may hereafter have arising out of, in any way whatsoever, any acts, omissions or negligence before, during or after my participation in such church sponsored ministries and activities on and/or away from church premises.

I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and assumption of risk agreement, and I sign it as an intentional act.

This release of liability and assumption of risk agreement shall remain effective until revoked in writing and delivered to an employee, agent or officer of Churches United, appointed to receive such documentation.

By entering my full name below, I affirm that I have read, understand and agree to the above Ministry Volunteer Liability Release, and also recognize that this is the equivalent of my legal signature.*

In exchange for serving as a ministry worker/volunteer or participant in a Churches United service or event, I grant to Churches United, its legal representatives and assigns, the right and permission to use my and/or my child's name and likeness and to use and publish photographic pictures, video, footage and sound recordings of me (the "Materials") and/or my child during church related events, whether via print, digital, broadcast or in any other media and for any lawful purpose.

I release, discharge and agree to hold Churches United harmless from any liability for any use of the materials. I understand that by providing my contact information listed above, I agree to be contacted by Churches United via mail, telephone, email or text message.
